A randomized controlled pilot trial led by David A. Kaminsky and colleagues from the University of Vermont College of Medicine, Baylor College of Medicine, and private yoga practices investigated whether 12 weeks of Dirgha Pranayama, a simple Three-Part Yoga Breath, could improve walking capacity and breathing-related outcomes. The findings suggest an intriguing role for accessible, home-based pranayama practice.
What if a simple breathing practice could help people become more physically active without requiring a complex yoga program? Researchers David A. Kaminsky and colleagues tested this question in a randomized controlled pilot trial involving 43 adults with symptomatic moderate-to-severe COPD. Participants received either education alone or education plus 12 weeks of Dirgha Pranayama, also known as the Three-Part Breath.
Dirgha was chosen because it is simple and emphasizes slow, relaxed, complete breathing. Sitting in a chair, participants inhaled slowly through the nose while visualizing the breath filling the lower, middle, and then upper lungs before completing a relaxed exhalation. The researchers proposed that this approach could encourage more complete lung emptying while also promoting relaxation.
After 12 weeks, the pranayama group increased its six-minute walk distance by approximately 28 meters, while the control group decreased by about 15 meters. The treatment effect narrowly missed conventional statistical significance (p=0.06), although the pranayama group walked significantly farther than the control group at the 12-week assessment. The pranayama group also showed small favorable changes in inspiratory capacity and measures related to air trapping. Participants practiced about 12 minutes per day on average, reported less shortness of breath in their diaries, and experienced no intervention-related adverse events. Because this was a small pilot study, larger trials are needed. Still, the findings suggest that a straightforward, home-practice breathing technique may offer a practical way to support exercise capacity.
